Upon the completion of the application, parents will be contacted to obtain/check

required documentation listed below.

For Registration to be complete, parents must have their child’s:

  • birth certificate (must be a state-certified copy and not a hospital mother’s copy)
  • Physical and Shot Record on TN School Immunization Certificate (white form)
  • Proof of Income
  • Social Security Card (Optional, but requested)

After the receipt of your digital registration, you will be contacted on how to supply the above documents.

Please note:

It is important that you complete all information in the digital registration packet so that the application process moves forward with a contact from a teacher to complete the information and document at least one form of income verification.

We will not be able to process any application for enrollment until these items have been provided and delay in receiving all paperwork could decrease your child’s chances in admission to the Pre-K program.

If your child is selected for the program, your child will need to have a physical examination and a completed Tennessee Department of Health Official Certificate of Immunization completed by your child’s health provider or the health department prior to the first day of school.  Your child will be unable to attend Pre-K until all required paperwork is submitted.  Due to new requirements in the Pre-K application process, students who do not meet income eligible status will not be accepted until 20 school days have passed even if there are spots available.

Please call your child’s zoned school or 745-1612 if you have any questions.
